Studio Terms, Reservation Fee & Model Release Form

The amount and information pertaining to your Photography Session and Package has already been provided to you by email. The email includes the package pricing and a confirmation of the date, time and location of your session. This form is to collect your acceptance of the Studio Terms, Reservation Fee and the Model Release.

Payment for services will be due before the session begins and is payable to: Christina Juleson by way of cash, money order or check*
*There is a $35 fee on all returned checks.

Reservation Fee & Refund Policy There is a reservation fee of $25.00 that reserves the day and time of the session. The time slot that will be reserved will no longer be available to other potential clients. For this reason, the reservation fee is non-refundable. If you do not show up to the session or cancel the session, the reservation fee will be retained by Christina Juleson. In the event of a medical emergency or inclement weather, making it impossible to provide the photography session outdoors, the session may be rescheduled by Christina Juleson to a different day & time and the reservation fee will be transferred to secure the time slot with the same terms outlined in this form. Christina Juleson reserves the right to cancel the session at any time for any reason and agrees to refund the $25.00 reservation fee to you.

Digital Images & Coverage Time
The session includes high-resolution digital files that will be available to you by direct download from your own personal website. You will have permission to use the images for your own personal use which includes: directly download, make prints & products and post to social media. The images may not be published or used in any instance where an exchange of money or monetary gain is concerned. Your session begins at the time that has been reserved, if you are late, the session will end on time.

Permissions and Use of Images
Client agrees that Christina Juleson reserves the right to produce samples from the session to be shown to prospective clients. Christina Juleson dba Christina Juleson Photography reserves the right to use, publish, and re-publish the photographs in whole or part in any medium including advertising, trade, art purpose, internet, social media, multi-media, or any other purpose. The Client hereby releases Christina Juleson dba Christina Juleson Photography and its legal representatives and resigns from all claims and liability relating to said photographs or any profit that may be made from the use of images from this event.
